You make an extremely important point here. My managers and board members never wanted to hear a negative or contrary opinion. As long as employees kept things light, cheerful and positive we all got rave reviews and great feedback, but as soon as anyone brought up problems or real issues, they were tagged as troublemakers or “not being a team player.” Your example of the space shuttle shows that the consequences of this kind of suppression of truth and outspokenness can be extreme.
